The Medal was instituted by a Decree of the Great National Assembly from 15 June, 1948.
It was awarded only once to participants in the June and September uprising of 1923.

It has the form of a circle with diameter of 28 mm.
The top right field on the averse bears the convex effigies of the leaders of the uprising, Vassil Kolarov and Georgi Dimitrov;
underlying in three lines is the inscription signifying: “23.IX.1923 ПОДГОТВИ 9.IX.1944”.
The left field bears the effigies a farmer with a sickle and a worker with a hammer;
underlying is the sketch of a gear-wheel and an anvil.
On the reverse, against the background of a mountain,
is the convex effigy of a group of rebels with a flying banner and a pulled-out gun.
Surmounting are laurel leaves, underlying on a ribbon is the inscription: “СЕПТЕМВРИ 1923 ГОДИНА”.
The Medal was worn on the left breast,
suspended from a triangular wine-red ribbon bordered in orange on both sides.
